Sadly it's a very large number of people who can't change their smoking habits despite their illnesses..

He's named Gherardo Fedrigo Gaetani dell' Aquila d' Aragona. Obviously he's an aristocrat as well, but I can't tell what title he bears...

His title should be Count Don Gherardo Fedrigo Gaetani dell’Aquila d’Aragona. However, I have some difficulties in finding him in the genealogy of Gaetani family.

Yes, Costantino is related to Count Gaddo della Gherardesca, though very distantly as they both descend from Count Guido Alberto della Gherardesca (1780-1854).
Instead, Costantino is more closely related to Countess Sveva della Gherardesca (the wife of Prince Nicholas Romanovich Romanov, the claimant to the Headship of the Russian Imperial Family); more precisely, Sveva is a first cousin of Costantino's father.
