The use of the long black dress was a very antiquated introduction of Sofia (maybe being a foreigner and not having been born a Catholic, she thought that highlighted her Spanishness), she herself in Franco's time came to a delivery of flag in a long red dress, and decades before the own Queen Victoria Eugenia had dispensed with the use of the mantilla in these acts.
The mantilla is only used in Easter and weddings, and is used more in Andalucia but very little in the rest of Spain. So it's normal for it to be something that Queen Letizia does not identify with, because it is something that most Spanish people of her generation do not identify with.

As for visits to the Vatican, they have always differentiated between state visits, where queens do not cover their heads, and religious visits where they do.
