Who is the young boy, between the ladies, with the Toisón de Oro around his neck?

It confused me because the caption says: "The wedding between María de las Mercedes de Borbón, princess of Asturias, daughter of Alfonso XII, and the infant Carlos de Borbón took place on February 14, 1901, it was one of the most important events in Spain that year."
But when the young boy on the picture indeed is the 5 years old Don Alfonso, then he was the Prince of Asturias and not his sister Doña Maria de las Merdeces.
He looks older than 5 years, I must say. But he has a Toisón de Oro around his neck, he must be one of the most senior Spanish royals. Maybe this was not a picture of the wedding?

Alfonso XIII was never Prince of Asturias, because he was King from his birth.
At the time of this marriage Alfonso was about 15 years old.

Yes, you are right. And on this picture Don Alfonso looked like 15-years old indeed. Interesting that the gentlemen around him wore the largest versions of their Orders (chains) and Don Alfonso kept it minimal. Maybe because he was still just an adolescent.

Don Carlos, Prince of Bourbon-Two Sicilies, Infante of Spain was the son of Prince Alfonso of the Two Sicilies, Count of Caserta and his wife Princess Maria Antonietta of Bourbon-Two Sicilies, and nephew of the last King of the Two Sicilies, Francis II.
Interestingly, he was the maternal grandfather of King Juan Carlos I. Prince Don Carlos and Maria de las Mercedes de Borbón had three children from this marriage (Alfonso, Fernando and Isabel). Maria de las Mercedes died while giving birth to her third child.
Don Carlos remarried on November 16, 1907, with Princess Louise of Orleans, daughter of Prince Philippe of Orleans, Count of Paris and Infanta Maria Isabel of Spain. The wedding took place in the UK. Don Carlos and Louise de Orleans had four children (Carlos, Maria de las Dores, Maria de las Mercedes and Maria de la Esperanza), with Louise being the maternal grandmother of King Juan Carlos.
