Perhaps the sort of formality that necessarily accompanies any events involving the Emperor and Empress makes them too stressful for her right now.

You are probably right. Crown Princess Masako and Crown Prince Naruhito gave a private dinner for British Crown Princely couple. That was a semi-official engagement carried out in the comforts of Japanese Crown Princely couple's house, which eased the level of stress for Crown Prince Masako. The same does not apply to the Spanish State visit, which is expected to be formal.
