It is an Official Visit, so a lower level of protocol and ceremionial (no State Banquet for an example).

Could she really have forgotten to take our her hat? In this case shouldn't the protocol responsible remind her ? Or it is made on purpose ?

In general a lady does not wear a hat in her own house, but -like Queen Beatrix did- for the welcoming ceremony outside at the Palace Cour, a hat was worn.

Usually in the evening a banquet is given but here the Greek guests were offered a lunch, so this was a day event. Maybe Queen Mxima, escorting the guests to the Gallery Chamber of Noordeinde Palace, forgot to remove her hat.

It also depends on the time. Queen Beatrix welcomed President Napolitano at daytime, wearing a hat: picture. Queen Beatrix said farewell to President Napolitano in the late afternoon/early evening, wearing no hat: picture.

i think, while true that in the past one would not have worn a hat at home, the royals go out to welcome their guests and then come inside. it would have been awkward for maxima to leave as soon as she got inside just to take her hat off: in fact, protocol and etiquette are all about 'making the other one feel comfortable and honoured' and i can think no worse way to do that than to disappear 5 minutes after meeting the guests.

so, no. i don't think she forgot. this is the proper way to do things, it would seem, at the dutch court. in fact, check out these pictures of other official visits:
