The family inner circle, very close friends who have known the Queen for many years, I would imagine. Not a lot of people.

King Juan Carlos is called "Juanito".

The royal children in Sweden has nicknames...

Victoria is called Oja by her family and old friends. The reason is that when her younger brother Carl Philip was little he couldn´t say Victoria, its sounded like "Toja" and then it became Oja
Carl Philip is called Lippy. I have no idea why :)
Madeleine is called Len by her friends, and that is imo the nickname that makes most sense: Madeleine...

Máxima: is called Max
Willem-Alexander: Máxima calls him Alex
Catharina-Amalia: is called Amalia (some argentinian press call her Amalita)

Frederik: is called Pingo
Mary Elizabeth: is called Mary

William: is called Wills
Hernry: is called Harry
