I don't think there is anything to worry about. HM is in good shape for a woman of her age. I think part of the reason the Queen has approved of Prince Charles's marriage to Mrs. Parker-Bowles though is the fact she knows that she is in the "sun set" of her reign. She is nearly 80 and I think she figures that even if she lives as long as her mother, she'll reign for another 20 years. By allowing Charles to marry while she is still in good health, the public will have time get "comfortable" to Prince Charles being married to Camilla. That way by the time he becomes King, everyone will be used to he and Camilla being a married couple.
The Queen has cut down on her duties, as mentioned above, but she has been cutting down on her duties slightly since the end of her Golden Jubilee in 2002. She has a number of engagments set for the summer. She'll be in Canada for over a week this coming May for example.
